First Visit
Thank you for choosing to see Dr. Stone. For new patients, please be sure to arrive 30 minutes prior to your scheduled time to allow for check-in, registration, and any necessary x-rays.
When you come for your appointment please remember to obtain and bring the following:
- Driver’s License or a valid ID
- Insurance information
- A CD (compact disc) with X-rays, MRIs, CT scans etc. and any other relevant information. It is important to bring a copy of the images and not just the report to aid in making an accurate diagnosis and avoid duplicate studies.
- Referral Letter (if required)
- List of medications (if any)
